It is simple: some providers give you a fixed IP, this will never change for you and than a ban will work.
Other providers work with an IP range (255 IP's per range) and every time you restart your router you get a different IP, than the ban don't work.
Some players who do have a fixed IP also use a proxy server. For the server manager it than seems to be a different IP so ban also don't work.
