@ Catalin - I'm not 100% sure on what causes this to happen with the eyes. It seems to always happen after getting blown up many times by hand grenades and other explosions, causing my dead body to take some flying lessons while it gets all twisted up in the air. I believe this is the cause of this; making the eyes appear in an unusual place like behind the head when you get respawned.
